1.  What is the name of your department?
   Diagnostic Imaging
2.  What administrative area does your department /area fall under?
   Our department has a department chairperson and we fall under the ER, OR,
   Cardio-pulmonary and ICU department at Flagstaff Medical.
3.  What is the management structure?
   Our department management is structured by a director, department   
   supervisor, charge nurse and lead tech.
4.  What type of radiology procedures requiring nursing care are performed at your hospital?  
   Radiology procedures requiring nursing at Flagstaff Medical are:
   MRI patients that need to be monitored and require IV sedation, CT biopsies, drain and tube
   placement, coronary angios (64 slice), US monitor patient needs
   SPECIAL PROCEDURES
   Procedures involve placing IVC filter, embolizations (uterine), TIPS (revisions), angiograms,
   angioplasty, nephrostomy tubes, stent placement, tunneled, PICC and various specialty
   catheter placements and disco gram.
5.  How many staff? (RN’s, RT’s, other) What credentials are required?
   Six (6) RN’s work in our department, four (4) are scheduled to work daily 35 technologists are
   employed by our department.
6.  How many MD’s are assigned on a daily basis?
   A group of ten (10) MD’s that work for Northern Arizona Radiology are assigned to our facility.
   Four (4) of the group are scheduled daily to work in the department.
7.  Who admits/preps the patient? Who recovers/discharges the patient?
   If our schedule allows the department preps and recovers the patients. We have three (3)
   available beds and help from Endo if needed.
8.  Who transports?
   Flagstaff medical has a transport team. A computer entry is made to request patient transport.
9.  Call (what hours, pay, response time, etc)
   Call starts when the last regular hour shift is complete. RN’s usually are on call once a week
   and every sixth week-end.
10. Current major issue?
   All department policies, guidelines and procedures are current
   Current major issues are non-existent at the time.
11. JC concerns?
   Flagstaff Medical Center in not JC accredited.
